What Is USPS Form 1583?
Anyone renting a mailing address through a Commercial Mail Receiving Agency (CMRA) — like Downtown Works — must complete this form. USPS keeps it on file to track who's using an address, in case that address is ever used for illegal purposes.
This is a USPS regulation, not a Downtown Works policy — we're required to collect it before activating mail service.
Identification Requirements
USPS requires two forms of ID — one from Section 1 (photo ID) and one from Section 2 (address verification). Credit cards, Social Security cards, and birth certificates are never accepted.
Important: The home address you enter must be your actual physical home address — not your Downtown Works mailbox address. This is required for USPS verification and compliance.
Fastest option: a passport for Section 1, and a U.S. state/territory/tribal driver's license or non-driver ID for Section 2.

A driver's/non-driver's ID can satisfy Section 1 or Section 2, but not both — you need two separate documents.
No other identification is accepted — no exceptions.

Is Form 1583 the Same as Mail Forwarding?
No. Form 1583 only authorizes Downtown Works to receive mail on your behalf — it doesn't forward anything. If you want the USPS itself to forward your mail, you'll need to file a separate change of address directly with USPS at usps.com.
Note: As a registered CMRA, USPS does not permit a change-of-address filing that redirects mail from our address to another location — this restriction applies specifically to CMRA and private mailbox addresses.
